Dental Plans

The dental plans that are currently offered by the Albany-Colonie Regional Chamber are provided below.

For members – with more than 50 employees – that are interested in health insurance coverage, call Kathy Giannantonio at (518) 431-1419.

Enrollment is effective the first day of the month following 30 days of membership (corporations); 60 days of membership (sole proprietors); or 30 days of employment (employees). If not enrolled during these periods – in most cases – you must wait until an open enrollment period.

$10 processing fee applicable and required with each application submitted.

American General Voluntary Dental Plan

Available for companies with between two and 49 employees.
Students are covered to age 19; or through age 25 with proof of full-time student status.
Employee coverage requires a minimum of 30 work hours per week; company tax and payroll documentation required.

Guardian Dental Plan

100% enrollment required for members with four or fewer employees.
75% enrollment required for members with five or more employees.
Available to companies with between 2 and 49 employees.
Students are covered to age 19; or through age 25 with proof of full-time student status.
Employee coverage requires a minimum of 35 work hours per week; company tax and payroll documentation required.

Blue Shield of Northeastern New York Dental

Blue Shield Voluntary Dental Plan

You must be enrolled in a Blue Shield Medical Plan to enroll in this voluntary dental plan.
Available to companies with between 2 and 49 employees
Students are covered to age 19; or through age 25 with proof of full-time student status.
Employee coverage requires a minimum of 20 work hours per week; company tax and payroll documentation required.
