The medical plans that are currently offered by the Albany-Colonie Regional Chamber are provided below.
The Albany-Colonie Regional Chamber's Health Benefit Program provides an opportunity for member companies to purchase health benefits coverage. Currently this is made up of medical/prescription and dental plans offered through various health insurance carriers.

If you are 65 or older and have or are considering Medicare coverage, you may want to view Medicare & You 2010 from the U.S. Department of Health and Human Services.
For members – with more than 50 employees – that are interested in health insurance coverage, call Kathy Giannantonio at (518) 431-1419.
Enrollment is effective the first day of the month following 30 days of membership (corporations); 60 days of membership (sole proprietors); or 30 days of employment (employees). If not enrolled during these periods – in most cases – you must wait until an open enrollment period.
Unless otherwise noted, employee coverage requires a minimum of 20 work hours per week; company tax and payroll documentation required.
Blue Shield of Northeastern New York
- Traditional Blue PPO 909 Plan
- Traditional Blue PPO 910 Plan
- Community Blue HMO 206 Plus Plans
$25/$25, $10/$40 or $20/$30 Co-payment Plans; $500 Inpatient Co-payment per Confinement
Traditional Blue plans require 50% of eligible employees to be enrolled in a Blue Shield plan. Only one idemnity plan per company. Community Blue plans require 80% of eligible employees to reside in the Blue Shield service area. Available to companies with fewer than 50 employees. Minimum annual gross income of $15,000 for sole proprietors. Students covered to age 19; or through age 25 with proof of full-time student status.
Empire Blue Cross
- Healthy NY HMO Plan with RX (Upstate only)
- Healthy NY HMO Plan without RX (Upstate only)

- Available to companies with 50 or fewer employees.
- Employer has not provided comprehensive health insurance to its employees during the past 12 months.
- If there was group insurance coverage, employer must not have contributed more than $50 per employee per month toward the premium.
- 30% of employees must earn $33,000 or less annually.
- Employer must contribute 50% of Healthy NY employee premium.
- A questionnaire must be completed, certified and approved before a member company and its employees will be enrolled for benefits.
- Individual/Sole Proprietor annual/monthly gross household income must fall within the limits established for this program.
- Available to a sole proprietor with a completed and signed application submitted for approval with the following required documentation: NYS proof of residence, proof of income and proof of employment. Contact the administrator at (518) 431-1419 and select option 1 for a complete list of acceptable documentation.
- Empire HMO Plan - Option 9 (Upstate only)
Available to active full-time employees working at least 20 hours per week and residing in Empire's operating area. Member company must have at least two active full-time employees and must have a bona fide New York address within Empire's operating area.
- Empire Direct Pay HMO Plan (Upstate only)
Available to a sole proprietor with a completed and signed application submitted for approval with the following required documentation: NYS proof of residence, proof of income and proof of employment. Contact the administrator at (518) 431-1419 and select option 1 for a complete list of acceptable documentation.
- Empire Total Blue with Health Savings Account (Upstate only)
Refer to plan information for requirements and further plan explanation. For companies with 2 to 50 employees. Sole proprietors are not eligible for this plan.
GHI Health Plan
- GHI HMO Value 25 Plan - HIGH PLAN (with $240 inpatient co-payment and new prescription drug plan)
- GHI HMO Value 25 Plan - LOW PLAN (with $500 inpatient co-payment and new prescription drug plan)
- GHI High Deductible PPO Plan 1 with HSA Option
- GHI High Deductible PPO Plan 2 with HSA Option

High Deductible Health Plans (HDHP) are available to corporations of two or more employees only. HDHPs have an option to include a health savings account. The High Deductible PPO Plans require a complete employee census with zip code to be submitted in order to receive a rate quote. Available to companies with fewer than 50 employees. Student covered to age 19; or through age 23 with proof of full-time student status.
Capital District Physicians Health Plan (CDPHP)
- AvidCare 15 Plan (with $500 inpatient co-payment)
- High Deductible PPO Plan B
Available to companies with fewer than 50 employees. Students covered to age 19; or through age 25 with proof of full-time student status. AvidCare 15 Plan is not available for July 1st open enrollment.
