Medical Plans
The Albany-Colonie Regional Chamber's Health Benefit Program provides an opportunity for member companies to purchase health benefits coverage. The insurance needs of each of our members is varied, and coverage options need to reflect this. To best meet the needs of our members, a single provider was selected who could offer products that would best meet the needs of our members. That provider is Capital District Physicians Health Plan (CDPHP).
An added benefit of moving to CDPHP is that the full menu of their products is available to each of your employees. That is, a company is no longer restricted to offering only one CDPHP choice. Your employees can select the product that best fits their individual needs, and the employer will still receive one bill from the Chamber. With CDPHP, all employees can also earn points worth up to $365 per year as a reward of better health management.

Vision and Dental Plans
Vision and Dental plans are also available through the Chamber. Offerings include Empire Blue Blue Vision, American General Dental and Guardian dental.

Medicare
If you are 65 or older and have or are considering Medicare coverage, you may want to view Medicare & You from the U.S. Department of Health and Human Services. Questions regarding Medicare enrollment or coverage should be directed to 1-800-MEDICARE. (1- 800-633-4227).
The Chamber offers group Medicare Advantage (Part C) plans. Please click the link below to download information on these plans. To qualify for one of our Medicare Advantage plans, you must already have Medicare Part A and B. For questions related to the Medicare Advantage plans we offer, contact Debbie Collett at (518) 431-1433. Note: Sole Proprietors are not eligible for the Medicare group product. Enrollment Information
Enrollment is effective the first day of the month following 30 days of membership (corporations); 60 days of membership (sole proprietors); or 30 days of employment (employees). If not enrolled during these periods – in most cases – you must wait until an open enrollment period.
Unless otherwise noted, employee coverage requires a minimum of 20 work hours per week; company tax and payroll documentation required.

NEW: The Patient Protection and Affordable Care Act (PPACA)
Passed into law in 2010, the PPACA requires that health insurers and self-insured group health plans provide a standard Summary of Benefits and Coverage form (SBC) to all individuals enrolling in medical coverage. Below are links to documentation about the SBC as well as all of the required forms for CDPHP plans offered by the Chamber. These links are provided to help our members comply with the law.
