Vision and Dental Plans and Rates for 2014
We offer a vision plan through Empire as well as dental plans with Guardian (small business and individual) and Delta Dental (Small Businesses Only - CDPHP).Empire Blue View Vision Dental Plans and Rates 2014
Current Guardian Dental subscribers: The Affordable Care act requires plans to provide Pediatric dental for children under the age of 19. If you have either the employee/children or family plan, you will automatically be moved to the ACA-compliant plan for 2014.
Current American General subscribers: AIG has made the decision not to accept new enrollment in their American General Dental plans. This decision does not affect in-force policies for these products. They will continue to offer renewals, and you still have the ability to add new insureds to your existing plan.
CDPHP Pediatric Dental Waiver Form *Pediatric Dental rider will be automatically added to those subscribers that have children under the age of 19. Medical premium rates will be as noted on the 2014 CDPHP Plan details and rates document above, PLUS $16.18 per child enrolled (up to a maximum of 3). If you have a standalone dental plan, you can sign this waiver for the rider to be removed.

The Chamber holds an open enrollment period every November for an effective date of January 1.
All new small business or sole proprietor enrollment applications must include either an NYS45 payroll document, a Schedule C, or a W4 issued to a new hire proving employment by the business.

Billing and Payment
Premiums for health insurance are invoiced on a monthly basis through our subsidiary, Business Services Corporation. Bills are mailed mid-month covering the following month, with payment due on the first of the month. (i.e., February premium invoice is mailed mid-January and is due February 1.) All payments are mailed to Business Services Corporation, PO Box 10145, Albany, NY 12201. Payments not received in a timely fashion incur a late fee of $25.

The Chamber offers a group Medicare Advantage (Part C) PPO Plan from CDPHP. Please click the link below to download information on this plan. To qualify for enrollment in our Medicare Advantage plan, you must already have Medicare Part A and B, and be considered a Retiree.
